I HEREBY APPLY FOR ACTIVE MEMBERSHIP IN THE ASSOCIATION, AND IF AN INVITATION IS ISSUED, I AGREE TO ACCEPT SUCH MEMBERSHIP, AND I HEREBY CERTIFY THAT: (a) I have read the Association's By-Laws and Code of Ethics, and agree to bind myself and my firm to conform to and uphold them and all official regulations, ideals and purposes of the Association. (b) Over the last thirty-six (36) months, I have generated aggregate gross commissions from leasing transactions in excess of $500,000 paid to my company. (c) I have functioned primarily as an office leasing broker or salesperson for at least sixty (60) months. (d) To the best of my knowledge, there are not now any outstanding judgments, lawsuits or bankruptcy or receivership actions pending against me or my firm, nor any other form of material challenge to my responsibilities, character or integrity, except as explained on the attached statement. (e) I irrevocably waive any claim, right of action at law or at equity which might arise hereafter against the Association, its directors, officers, committee members or other officials, either as a group or individuals, for any act in connection with the business of the Association and, in particular, those acts concerning my admission to membership, in disciplining me as a member, or related to any arbitration.
